Charge Nurse, Care Lead
•8 minReview a resident risk signal
A signal accepted, dismissed, or escalated with a written reason in the audit trail.
Before you start
- Care Lead role or higher
- At least one open resident signal
Steps
- 1
Open the resident from Morning Row
The Morning Row card links straight to the resident's Risk view with the relevant signal expanded.
- 2
Read the source trail
The trail lists every signal, assessment, and import event that influenced the current risk level. Read this before deciding — it's the model's evidence.
- 3
Check confidence and review status
Low confidence means the model is leaning on thin signals. That's a cue to gather more information before escalating, not to ignore the card.
- 4
Choose accept, dismiss, or escalate
Accept logs the recommendation. Dismiss closes it (with a reason). Escalate creates a follow-up task on the resident and notifies high-risk subscribers.
Common pitfalls
- • Dismissing without a reason is allowed but discouraged — the survey trail will show empty rationale.
- • Escalation does not page on-call by itself; configure that in notification thresholds.
